What’s the difference between an Insurance Carrier and an Insurance Plan?

• Insurance Carrier is the company (e.g., MetLife, Delta Dental, Aetna, Cigna).

• Insurance Plan refers to your specific coverage under that carrier (e.g., PPO, HMO, Advantage).

Note: A provider may be in-network with your insurance carrier but not necessarily with your specific plan.

What is a Deductible vs. a Copay?

• Deductible: What you pay out-of-pocket before insurance starts covering costs.

• Copay/Coinsurance: The portion of the cost you pay after meeting your deductible.

What does my insurance cover?

Coverage depends on the agreement between you, your employer, and the insurer—not the dental office. Most plans fully cover preventive care (cleanings, exams, X-rays) even before the deductible is met.

As an in-network provider, we honor the contracted rates negotiated with your plan.

How can I check my out-of-pocket cost?

You can:

• Call your insurance carrier directly.

• Contact our office for an estimated breakdown of fees based on your plan.

What happens if my insurance denies a claim?

While we do our best to verify your benefits in advance, any denied payments are the responsibility of the patient or guardian.

How do I find an in-network provider?

Visit your insurance provider’s website to see your preferred provider list.
Keep in mind: Your plan may be out-of-network even if the dentist is in-network with the insurance carrier.

What if our office is out-of-network for your plan?

Your insurance still provides maximum allowable rates for out-of-network care. In most cases, this means your out-of-pocket costs won’t increase significantly.
